At this non-medical meeting, you can meet Dr. Tom and decide if the practice is right for you. If you decide to join, we will have you read and sign a membership agreement and enrollment form, and release of medical records as needed. Please note that we will contact you by phone prior to your appointment to confirm all the details and answer any questions you may have before coming in.
